Whilst the greatest effort has been made to ensure the quality of this text, due to the historical nature of this content, in some rare cases there may be minor issues with legibility. To make an index to a work of this kind is almost an impossibility. In place of attempting the impossible, I have given in the last chapter a brief summary of the whole book, with reference to chapters. This will serve also as a detailed table of contents.
