The goal of endodontic and periodontal therapy is to preserve natural dentition and restore lost periodontium. The relationship between pulpal and periodontal diseases, highlighting their embryonic connection through the mesodermal-derived dental papilla (pulp precursor) and dental sac (periodontal ligament precursor). Anatomical pathways such as the apical foramen, accessory canals, and dentinal tubules allow communication between these tissues, facilitating the exchange of pathogens and inflammatory mediators.Pulpal disease can trigger periodontal inflammation via pathways like the apical foramen, leading to epithelial proliferation and tissue breakdown. Conversely, periodontal disease may affect the pulp when accessory canals are exposed, allowing pathogens to cause chronic inflammation and necrosis. However, as long as the apical vasculature remains intact, the pulp may retain vitality.Endodontic infections can exacerbate periodontitis by promoting epithelial downgrowth and connective tissue loss. Treating pulpal infections is critical before addressing periodontal disease, as this reduces the risk of progression and facilitates tissue.
