Whilst the greatest effort has been made to ensure the quality of this text, due to the historical nature of this content, in some rare cases there may be minor issues with legibility. When the Council of the Society did me the honour of suggesting that this ancient and unpublished document, together with my notes and explanations, should form the subject of a work to be issued by the Royal Society of Literature as one of their special publications, I gladly fell in with the proposal, and the volume now presented to the Fellows is the result.
