Whilst the greatest effort has been made to ensure the quality of this text, due to the historical nature of this content, in some rare cases there may be minor issues with legibility. Believing it to be the better plan, reading references to easily obtainable books have been given at the end of each chapter, rather than a more complete bibliography containing books practically unobtainable. For this reason also only books in English have been cited.
